These forms are mostly for the Boy Scout program. Forms that are specific to Cub Scouts, OA, District or Venturing may be found in that spicific area. Finally, some of these forms may have been superceded.  If a mistake or a form that you would like to see is not here please email me the information and I will attempt to find the missing form. Also if you have a new or updated form to add please send me the link and it will be added. Unless noted otherwise all forms are in Adobe Acrobat Format.  

Sometimes we merely link to a National Form, if so it is labeled as such. Sometimes our form is better, but if you want the National form link it is also listed.

Boy Scout Advancement

First Class First Year Tracking Sheet Lists requirements and where they can be done in category groups. Use one sheet per scout. (12kb) 2000  BSA # 34118

Individual History Use this form to track advancement for each Boy Scout. Covers rank, leadership, OA, and special awards.  (11kb) 2000  This form is not needed if you use any of the Computer tracking programs on the market.

Boy Scout Program

Application for Campership  This 9801 form is filled out when fincaial assistance is needed for a youth to go to camp. Please follow the included instructions. Two copies will be needed. (87 kb)

Congressional Awards for Scouts This is an award for Boy's 14 and older which deals with volunteer public service, personal development, physical fitness and expedition/exploration activities. (52kb) 2000

Boy Scout Insignia Placement Shows the Scout uniform and proper placement of most patches. If you do not see what you need get a copy of the Insignia Guide from your Council Service Center. BSA #  34283 National Link     &

Troop Resource Survey Hand this form out to the parents to see what skills they have that might help the Troop.  Give every adult at least 1 small job that they can accomplish. Next year they will be able to do the bigger jobs.  (312kb) 2000 BSA # 34437

World Conservation Award for Boy Scouts.  If a Scout earns Environmental Science, Citizen in the World and either Soil and Water Conservation or Fish and Wildlife Management he is eligible for this award. (28kb)  This form is able to be filled out using Adobe Acrobat.  ?

Baden-Powell Patrol Award This form is used to see if your Patrol and Patrol members are following the Scouting Program. If you do then there is a special patch that you can wear under your Patrol Emblem (67 Kb)

Camp Site Inspection Use this form for a troop camping trip. Set up to judge 4 patrols on all aspects of a good campsite. (23kb)

Menu Planner This is a helpful form for your camping trips. Assists in meal planning for a weekend camping trip. (5kb)

National Camping Award   is designed to recognize troops who go camping during the year. Your troop may qualify in two categories as you begin to expand your camping program: yearly and cumulative. You can also qualify for an individual award. This form lists all requirements for all categories. (71 kb)

Conservation Awards

Eagle Scouts

Application for Alternate Eagle Award Merit Badges  Occasionally there is a Scout that cannot complete the list of required merit badges due to medical reasons. This form is needed to get permission for alternate badges. Once form is filled out it is filed with your District or Council  Advancement Chairman for processing. Eagle leadership Project Workbook This is a workbook that is to be filled out for your Eagle Project.  It is now acceptable to do it completely on the computer but it must follow this format. Spirit of the Eagle The Boy Scouts of America has created the Spirit of the Eagle Award as an honorary posthumous special recognition for a registered youth member who has lost his or her life in a tragic accident or through illness. Leaders Training &Recognition

For each adult job in scouting there is a training award that can be earned. This is a certificate  and a colored square knot that can be sewn above the left pocket. The requirements for each award are found in the Leadership Training Committee Guide (34169),  you can get copies from  the Council Service Center, or you can get pocket guides to the requirements for each award below. 

Boy Scout Leader Training Award  (GSLAC only) This is for any Boy Scout leader.  It can be the Scoutmaster, an assistant or a committee member.   (30 kb) 1996   BSA # 34169-55  National form link

Boy Scout  Leader Inventory of Training  Each Committee Chairperson should fill this out to keep track of leader training. (5kb) 2002   Excel

Commissioner Key/Arrowhead Award  This is for Unit Commissioners, to track their progress for this award. (25 kb) 1999  BSA # 34169-63

District Award of Merit form for 2008  This is the North Star District Form (only).

District Committee Responsibility Cards These cards describe the responsibilities of each of the key members of a standard scout district. (62 kb)  2001  34266D

District Summary of Trained Leaders This form is used by the District training committee to track the percentage of leaders in each unit that are trained.  (7 kb)

Scoutmaster Award of Merit Nomination  This form is for a Scoutmaster that has done his job as outlined in Scout Leader Training.  This is the qualifications and application for this award. Medical Forms

Class 1& 2  Medical Form  Class 1 form is used for things like  troop records and short term camps. Class 2 requires a doctor’s exam and is needed for most long term camping.(19 kb) 1999. BSA # 34414A  National Link

Tour Permits and Permission Forms

Local Tour Permit Application This form is for local trips. Contact your local Counsel for rules as to when this form needs to be filled out. Military Campways Information and Reservation Form Use this form to request information from any military base.
